This screen test is a short assessment tool to identify whether you are a candidate for formalised ADHD assessment with a Clinical Psychologist or Psychiatrist.

This screen test will help to:  
self-rate yourself to determine the behaviours that may align with ADHD
allow your partner to assess you from an external standpoint as to what they witness around your behaviours.
 
Do the screener twice using the same sheet of paper using different colour pens - one colour for yourself and a different colour for your partner's rating of your behaviours.
